Enrollment Information - School Withdrawals - Transcripts
During the school year enrollments are done by appointment only and we usually don't enroll students the first few days of school.
7th Grade Information
Documents Required to Enroll a 7th Grade Student
Documentation of the Child’s Birth – A copy of: State Certified Birth Certificate, or Legal court record containing this information. Please no abstract Birth Certificates.
Immunization Record – A complete and legible copy of your child’s Immunization record, state immunization card or medical doctor immunization record.
Please be sure that your child’s name and birth date are clearly printed on their immunization record. Note there is a new state requirement: Tdap Pertussis
ALL students entering a California Junior High should have completed the following:
Tdap Pertussis booster one dose on or after the 10th birthday (new state requirement)
Hepatitis B vaccination (Series of 2 or series of 3 vaccinations, doctor must state series
type.)
MMR (Measles/Mumps/Rubella) vaccination (minimum of 2 are required)
TB Skin Test (PPD-Mantoux) should have both the “Date Given”, “Date Read” as well as the Impression type (Positive/Negative) documented on the Immunization record.
Proof of Residency – Please bring originals of at least three items showing your residency address such as utility bills, house insurance, escrow papers, property tax bill, lease / rental agreement, vehicle registration. Originals will be photo copied and given back to you.
NO STUDENT WILL BE SCHEDULED INTO CLASSES WITHOUT PROOF OF IMMUNIZATIONS
8th Grade Information
Documents Required to Enroll an 8th Grade Student
Transcript – With all course grades for 7th grade and any course grades completed in 8th grade.
STAR or State Test Scores
